Statement of Purpose The purpose of this survey is to honor individuals and teams that have provided significant customer service to states and communities through the Flood Risk Management and Silver Jackets Programs. Participation in this survey is completely voluntary. Responses will be anonymous. Reports generated with these evaluations will show impacts only as aggregated by broad categories (such as commercial, industrial, public, and residential), no information will be released that can be used to identify you or your facility. The information collected will be managed in accordance with AR 25-400-2 records retention requirements.
